A custom thumbnail versus an auto-generated frame is the highest-leverage item on this list — YouTube picks that auto frame algorithmically, with no guarantee it's a flattering or even coherent moment from the video, and thumbnail plus title together are what most people actually decide to click on.
Reviewing auto-captions for accuracy rather than trusting them outright is the other one — misheard words in captions are both a viewer-facing embarrassment and something YouTube's own systems can use when indexing the video's content, so a wrong word there can quietly cost more than it looks like.
